I just installed and set up a IW5M dedi server, and whenever I start the .exe it says "Spawning server on map mp_dome and gametype war" and shortly after it comes up with "iw5m_server.exe has stopped working." Any ideas? Thanks.

The server isn't being able to create the required connections during the expected time.

Make sure that the server ports are forwarded and that no firewall/anti-virus is messing with it.

A workaround that works sometimes is to remove "+start_map_rotate" from the startup arguments, start the server and then input the command a few seconds after the server launched.
