How do you show server status in signature?

Hey Guys,

 

I run multiple servers, how do you show server status like this guy has in his signature here

 

memberlist.php?mode=viewprofile&u=24268

 

I have a basic service status page which just checks if the port is open and in use but nothing special, just wondering how you get it to show how many active players there are on the server, I presume i need a little script that connects to my DB to get the number of active connections?

 

This would be a nice feature to add to my website and signature on the forums if anyone knows how to do it.
