How about re-installing the game?

Delete your IW4M folder run the updater, and if that didn't work try re-installing the whole game.

At the post '' Bug fixes etcetera '' Can you post me a good one if possible? Or gimme yours.

That one does work, try renaming your "miles" folder to "miles - old" try to launch the game then. It's not a fix but it might be a work around.
