The only reason you ever need to forward ports with a router in the first place is to get through NAT. If you're connecting directly to your cable modem, you don't have to forward through anything. You're already on your external IP.

 

You are talking about hosting a server on your PC, and then connecting to the server with a client on that same PC, right?

Yes, same IP. Both the server's and the client's (The same PC)

I've setup more than 1 server with direct connection and never even had problems with those servers.

All I can say is yes, you can setup your servers with just a modem. If you run into problems, check your server settings (you won't have router/port forward problems) lol
