/quit in console and restart the game once or twice

It's a known issue, just restart the game.

1) Launching the LaunchIW4M.exe is enough

Had the same problem, restarted the game, then got this: Fatal error (0xc0000005) at 0x006bdb59. A minidump has been written to iw4m-3.0-105-20120906164506.dmp.

The r_noborder windowed mode is full screen too. (thank you captain obvious)

in you commandline to enable it.

just write 'readstats' in console to fix your problem

Set it lower manually then and it will be running fine
