I'm getting this error at revision 88:

Creating Direct3D device...
Couldn't create a Direct3D device: Device lost
********** DirectX returned an unrecoverable error code during initialization  **********
********** Initialization also happens while playing if DirectX loses a device **********

Any idea?

Yes, Make sure that before you run iw4m.exe there is no process with that name running. So kill the process if it exists and then start iw4m.exe again.
