Steam bans have nothing to do with 4D1. You can play here, as long as you don't cheat.

As for private matches, other people can connect using your IP (and yes, you have to forward port 28960 for private matches).

As for dedicated servers, you can either forward some ports and run it yourself, or you can rent a VPS.
